A vital part of the multidisciplinary
team, the Social Services Department at JARMC is staffed by 3
licensed social workers. From the nursery to our geriatric patients,
the Social Work Department addresses the social and psychological
problems related to illness. Through a screening process, patient
needs are identified and assessed.
Discharge Planning
Discharge planning, a large part of the social worker's role,
begins on admission. This can include assistance with nursing
home placement, swing bed, rehabilitation, personal care, home
IV-therapy, home health, or hospice. Social Services can also
assist with any home equipment needs to ensure continuity of care.
Utilization of Community
Resources
Assisting patients and families with emotional impact of illness
is an important role of the social worker. Referrals to community
agencies for meal assistance, transportation, housing and financial
assistance are also provided. Advance Directive information packets
are available upon request.
Counseling for Patients
and Family
Crisis intervention is provided for suicidal situations, abuse,
neglect or mental illness. A Social Worker is on-call after hours
to consult with staff in crisis situations.
